Usually, you can't tell by looking at a man's penis or genital area whether or not he has an STD. The exceptions would be genital warts (HPV), or if the penis is red and/or has a discharge from gonorrhea or whatever, or genital herpes during an outbreak. Other than using a condom, you can't be 100% safe. However, you can take some common-sense precautions.
Don't perform oral sex if you have cuts in your mouth, abraded areas or bleeding gums. Don't receive oral sex from a man who has cold sores or any sort of infected looking sore on his mouth or tongue. Know your partner. The more random partners you have, the greater the risk of getting an STD.
If you're not going to use a condom, your best defense is knowing your partner. Knowing that he's not promiscuous. Not being promiscuous yourself. Sucking off random guys in club bathrooms makes for hot porn, but in reality it's just too damned risky.
