Apart from getting him to see a doctor, my suggestion would be to do your best to get him going out for a walk, twice a day would be good and taking a shower first thing in the morning and again at night, a shave too. The little things are what will make the most difference here.

It works on so many different levels, gets him out and about so he can see the world is not so bad as his mind is painting it, gets him in a routine, stops him sitting dwelling on things, works up a hunger so wants to eat etc etc
