That doesn't really happen unless someone's poked a needle in the tip... and if so, the condom might have ripped earlier from all the wear and tear (coming and going)...
On a serious note, that is precisely why it is advised to pinch the tip of the condom before rolling it down onto the head, as it will act as a reservoir and be easier to keep the ejaculated semen inside the condom for disposal.
