Well, they are sensitive...
A - when it occurred you were in the moment and ignored the pain.
B - Only advice I have is to read through this list and consult a doctor if it seems appropriate. The biggest issue is torsion, or where a ball might have been twisted around. That's a serious condition, and needs immediate attention.
C - Can't really guess at that. They are sensitive. My fiance has one of his balls stitched to keep it in place. Any type of play causes him discomfort. Easy to forget in the moment, and even a brush against it can cause him to react.
