I agree with those who say 'Don't lie'.

I agree with Zeon, this is a 'selling point' if worded correctly. You have a unique perspective on an issue that millions suffer with thus may be able to do more to save lives than 'normal' people who never been there and are actually clueless about the matter.

I doubt that questionnaire will give you the space to present this as a positive.

I agree with Fred, its difficult for them to discriminate over a 'disability'. Its real easy for them to drag you through the coals of hell over a lie - regardless of its colour.
